If I gave my 20 lb dog a flea medicine for a 5-10 lb dog would it have the same effectiveness?

I went to the vet and said i needed 5 things of flea medicine and they gave me a box for 5-10 lb dog, i have 3 that weigh in that range but i have one that weighs about 15 lbs and another that weighs 20 lbs, i guess i could go back and exchange but I was just wondering if it would make a big difference to give a smaller dose

Yes, it does make a difference or the company wouldn't sell different sizes. Take them back and exchange them for the appropriate size.
